What to Bring to Your Appointment
- Your new patient forms
- Your insurance card and photo i.d.
- A list of current prescription or over-the-counter medications you are taking, including dose and frequency.

Payment
- Payment may be made with cash, check or credit card.
- We take all credit card payments (American Express, Discover, Mastercard & Visa)
Insurance
We are happy to process all insurance forms for you as long as you provide us with appropriate insurance information. All copays and deductibles are due at the time of service. If you have any questions or concerns regarding your insurance plan, billing or financial arrangements, please contact our office direct at 205.206.4518.
St Clair Eye PLLC is a provider for most major insurance plans. However, it is in your best interest to check your insurance provider's list of participating physicians to be sure your plan allows you to be seen at our office. Also, please review with your insurance customer service representative (or insurance provider) the rules and limitations on coverage and reimbursement.

Other Office Policies
Late Policy: Patients are given a 20-minute grace period from the time of the scheduled appointment. If patients have not checked in by that time, they will be rescheduled for another date and time.
No Show Policy: Patients will be charged a $20 fee if they do not show up for their scheduled appointment.
